I just went to PRC head office today to change my registered name because I got married last year. I will also renew my license for the first time after 2 years of expiration. It was a like a battle field there, I think PRC’s one of the government agencies that has never achieved a good change, since 2006 when I was filing for a licensure examination until this year, 2013, when I need to change my name and renew it. Well, other government agencies I’ve been to like NSO, BIR and DFA have improved services. I’ve been there for 3 hours, battling the heat, applicants and endless lines.
1 pm, my time, I entered PRC grounds and asked the guards the process on changing my registered name or “change status” as they mentioned and to renew my license at the same time. Here’s my own steps how (this is for Nurses, I’m not sure if the process is the same with other professional licenses):
You will need to bring:
- Marriage Certificate in security paper (SecPa) and 1 photocopy
- PRC license and 1 photocopy
- 2 pieces passport size picture with complete name tag and white background
- pen and paste
- Go to “Public Assistance” which is right on the left side when you enter the gate to get form for renewal. Or you can download it here.
- Go to 2nd floor of the Main Building and the first door to your right. Get a form for change status/ change of registered name due to marriage or download it here. (I was there at 1:05 pm and the guard told me to wait because the personnel on that area is still on break. They resume at 1:15 pm)
- Fill up your forms and attach your passport sized pictures.
- Go to the Legal Division, same floor same building, have your Change of Registered Name form notarized for Php 50. (I had no money left on my pocket but my 1000 peso bill, the lady told me there’s no change without even looking inside the box where she puts the money. So, it’s all okay, I went outside and had my lunch first since I’m also hungry and thirsty at the same time because I went to my alma mater earlier that day to get something fixed and went straight to PRC. After 20 minutes, I went back and gave the lady the exact amount, much to my surprise when she opened the box, it has lots of hundreds and fifty peso bills. Duh! Why tell me you have no change when you do, I think that’s just crazy!)
- Go to Window 16, which is on ground floor and give your papers to the personnel. You should have 4 papers with you (The 2 PRC forms and photocopies of MC and PRC license). Make sure you’ve completed the forms and attached a copy of your Marriage Certificate and PRC license. (They will just double check if you have an original of both. The guy on this window is very helpful).
- Go to Window H “Records Section”, which is outside near the entrance gate, for the verification of your examination records.
- Go to Window O or P to have your petition form metered. It costs Php 21.
- Pay at the cashier. For my renewal, I need to pay Php 990 because I wasn’t able to renew in 2 years since 2010. There’s also a statutory fee of Php 225.
- Go back to Main Building 2nd floor and submit your forms. (It’s the room where you got the change status form). Don’t forget to get your claim slip and you’re done!
- By the way, you will get your license bearing your new name after 2-3 months and there’s no express or rush processing. Sad.

Thanks, your post is informative. By the way I’m in the US right now and soon planning to renew and change my status at the same time (nurse license). But I got married here in US. So I don’t have marriage certificate from NSO/security paper. My mother will be my representative. Do you think they’ll still consider my marriage contract from here? hi. Just like i just renewed my license as well as changed my status to married. I paid fees for both request and told me that I will be able to claim my id in 6-8 months. Wow, thats too long for me that is why i asked if i can check it in Manila after 3-4 months bt could not give me a solid answer that is why i was quite pissed off, that is why upon reading your post that you will be able to get your id within 2-3 months I am now sure that i will be able to have my id within that time and no need to wait that long. I am here in Baguio. Cant understand why it will take that long for the printing and updating of licenses when they can release a renewed prc id in a day as long as you pay an additional 450! I mean what’s the difference, right? They will just be printing it and updating their records in the computer so it should be far more convenient. And even if we have a petition for change of status, it should not still take that long for the id to be processed, right?? Maybe it is ok if it will just take a month or two but 6-8 months??Cmon!! Well, pardon me for bringing out my concerns here. I was just quite unsatisfied with the process.

That I’m not sure of, but I think they have courier services. When you give them the form, just ask if they can have it delivered to you in the U.S.

The renewal form should bear your married name, there’s a separate form you need to fill out “Petition for Change of Registered Name due to Marriage”. The photos should also bear your married name.

They calculate it every year that you’ve missed. I don’t know the exact amount but I paid 990 for a lapse of 3 years plus 225 statutory fee.

Hello Kitchen Anonymous, Nagparenew din ako ng license ko at nagpachange status/name. Grabe, ang experience ko 7am ako dumating. 1st nagpanotary ako pagdating ko ng 7am nanotarized agad petition papers ko sa 2nd flr main bldg Legal Division. Sabi sa akin don punta daw ako sa Window 16. Nagfill-out pa ako ng renewal form na maliit aside don sa nadownload ko online sa guard or sa window 18 meron non. Tagal ko naghintay bago sila nagbukas ng 8am. Sarado na yung window 16 kaya sa window 18 ako pumila. Tapos nung ako sa Windows 18, sabi sa akin sa Window 28 daw nilipat yung verification ng papers at assessment kung magkano babayaran ko. Nung pumunta ako sa Window 28 sarado pa past 8am na yon hah. Mga 8:10am pa kinuha yung papers ko ng old lady don tapos. Pinabalik sa akin yung maliit na renewal paper sa akin sa Windows 18. Tapos hinintay ko sa Windows 28 yung papers ko petition form, photocopies ng MC at license ko, Orig na MC at assessment kung magkano babayaran ko. 8:40am na binigay sa akin. Pero kung makikita mo sa loob kwentuhan lang at lakad lakad lang yung nag-assist sa akin sa loob. Tapos diretso sa na ako sa Window H, metered stamp then bayad na ako sa Cashier. Bumalik pala ako sa Windows 28 para makita kung san ko ipapasa yung completed prc form ko. Nakita ko pa rin don yung kasabay kong nagpasa ng papers nya. 9:08am na yon hah, 2nd pa lang yung girl na yon 9:08am ganon kabagal yung nag-asist sa amin kasabay ko lang sya ng 8:10am. 855 lahat binayaran ko kasama na yung 255 fee at yung bayad sa renewal. Tapos pinasa ko na ulit sa Regulations division yung forms lahat tpos binalik yung orig MC ko. Swerte kc mabilis lang process mo nag parenew at change of name. It took me 2hours. Kung mabilis yung babae sa Window 28 cguro 1hr lang tapos na ako. hay.

Yes, the processing depends on the person who assists you and I hate them PRC personnels because they are not punctual. Remember when I went to 2nd floor to get the form for change status? It says there office time resumes at 1pm after lunch break but they got back at 1:15pm. And as I’ve said, I’ve processed my documents for 3 hours! That makes me unlucky, too.
🙂 Anyways, I think PRC is one of the governement agencies which has not achieved a good change. Good thing is you’ll now have uour new I.D. – well after 3 months, that is!

Yes, you need 2 forms both the renewal (check step 1) and the Petition for change of name form (step 2).

wow. you are lucky you got your license immediately. I renewed and applied for a change of status last year (Oct.2012) and they said I can come back after 8mos-1yr. I went back last month (Sept.2013) having high hopes that my new license is ready. I was so surprised and depressed that till now my maiden name is still the one recorded on their system. when I asked them on how long should I wait, the person in charge said they dont know. They are still working on other renewals and change of status applications and they cant give me a definite time for me to come back. really annoying 😦 .

My question lang po ko regarding sa change status. Nag file na po ako nitong tuesday lang. Anyway ang question ko po, pati ba yun middle name mababago? Or the same padin kagaya nun hindi pa married. Worried lang po ako kase nun nasa window 28 ako at binigay na yun papers ko na finill up nila yun petition. Nakacross out na yun dati kung lastname at sa taas nakalagay yun bago na. Pero napansin ko hindi naman nacross out middle name ko para maging dati kong last name. Hindi ko napo naitanong ito 😥
Kaya nasstress out ako baka ang lumabas sa prc license ko ay yun middle name ko ganun padin.
Ask ko po ganun din po ba nangyari sainyo? Yun last name lang nacross out at pinalitan lang ng married name.
